Purpose. The template used to break a task into steps, identify the hazards of each step and record the controls before high risk work starts.
1.When a job safety analysis is required
A job safety analysis must be completed before any high risk work as defined in the Work Health and Safety Policy, before any task that is new to the site or the crew, and before any task where the existing work instruction does not fit the conditions. A current analysis will be reviewed at the start of each shift by the crew doing the work.
2.Breaking the task into steps
List the steps in the order they will be done. Keep each step to a single action. Include set up, the work itself, testing and clean up.
3.Identifying hazards
For each step, record what can cause harm to people, property or the environment. Consider energy sources, explosives and dangerous goods, plant movement, height, confined spaces, weather and fatigue.
4.Controls
For each hazard, record the control using the hierarchy of controls, starting with elimination. Personal protective equipment is the last control considered, never the only one. Name the person responsible for each control.
5.Sign on
Every person doing the task must read the analysis and sign on before starting. The supervisor must sign the analysis as approved. If conditions change, stop and revise the analysis.
