The situation

What has happened

Flight simulator FS-02 was withdrawn from training this morning with a fault on the pitch actuator of the motion system. The customer has a full training day booked tomorrow and the contract requires a priority 1 response within four hours. A spare actuator is on site. Ahmed Saleh raised the work order and completed the initial fault diagnosis before being called to another job. Entering the motion envelope is high risk work and the motion base must be pinned and the hydraulic supply isolated before anyone goes underneath. Fiona Kaczmarek wants the repair done, tested and recorded so the availability figures in the monthly report are supported.

Your brief. Take over the open work order, plan and carry out the actuator replacement safely, test the motion system against the acceptance criteria in the work instruction, and close the work order with a complete record of parts, tests and time so the simulator can be returned to training.

What to look for

Evidence guide

Evidence must show isolation and pinning before entry, a job safety analysis that names the person responsible for each control, parts recorded with batch, test results recorded against acceptance criteria and a closed work order that meets every closing requirement in the procedure. The outage period must be calculated from the work order timestamps, not estimated.
