Monthly Contract Performance Report Template

Purpose. The standard format for the monthly performance report submitted to the customer under each performance based contract.
1.Purpose
The monthly contract performance report is the primary evidence that the business has met its obligations for the period. It is due to the customer by the tenth working day of the following month and must be prepared from records in the Contract Deliverables Tracker, the Maintenance Work Orders system and the site registers.
2.Structure
The report will contain an executive summary, a performance measure table, a deliverables status table, a cost summary, a safety and security summary and an issues and risks section. Each section must reference the source records used so that the customer can verify any figure.

3.Performance measures
Each measure must be reported as the actual result, the target and the variance, with a short explanation for any measure below target. Availability and response time figures must be calculated from closed work orders only.
4.Costs
Costs will be reported by cost centre against the approved budget for the month and year to date. Any variance greater than five percent must be explained. Figures must reconcile to the finance system before the report is issued.
5.Approval
The department manager for each service line will confirm their section, the Contracts and Finance Manager will check the whole report and the General Manager will approve it before submission. The approved report will be filed against the contract in the tracker.
