Incident Reporting and Investigation Procedure

Purpose. Sets out how incidents, near misses, hazards and security events are reported, recorded, investigated and closed at every site.
1.Immediate actions
The first priority is the safety of people. The person who discovers an incident must make the area safe if it is safe to do so, provide or call for first aid, and notify their supervisor immediately. An explosives incident must be reported to the Explosive Ordnance Services Manager before any item is moved.
2.Reporting
Every incident, injury, near miss, hazard, property damage event and security event must be recorded in the WHS Incident and Hazard Register before the end of the shift in which it occurred, using the Incident and Hazard Report Form as the source record. The supervisor must review the entry and set the severity. Notifiable incidents must be reported to the regulator by the Safety, Security and Compliance Manager immediately.

3.Investigation
Incidents with a severity of moderate or above, and every explosives or security incident, will be investigated by a trained investigator appointed by the Safety, Security and Compliance Manager. The investigator will gather evidence, interview those involved, identify the contributing factors and the root cause and record findings in the register within ten working days.
4.Corrective actions
Each finding must be matched with a corrective action assigned to a named person with a due date. Actions will be selected using the hierarchy of controls. The register status will remain Actions open until every action has been completed and verified.
5.Communication
Lessons from investigations will be shared at the next site safety brief and, where relevant, across all sites through a safety alert. The customer will be informed of any incident that affects their people, materiel or facilities within the contract timeframe.
6.Review
Incident and hazard trends will be reviewed at the monthly health and safety committee and the management review. Repeated incidents of the same type will trigger a review of the relevant procedure.
