Purpose. The source record for any incident, injury, near miss, hazard or property damage event at a Sentinel site or in Sentinel custody.

1.When to use this form

Complete this form for every incident, injury, near miss, hazard and property damage event, including events involving contractors and visitors. Complete it as soon as the area is safe and before the end of the shift. If you are not sure whether something is an incident, report it.

2.Who completes it

The person involved or the person who discovered the event completes sections one to four. The supervisor completes the supervisor review and sets the initial severity. The Safety, Security and Compliance team completes the investigation and closure sections.

3.Describing the event

Write what happened in plain words in the order it happened. Include the location, the equipment or materials involved, the conditions and who was present. Do not record opinions about blame.

4.Explosives and security events

For an explosives event, record the nature, lot number and storehouse and confirm that nothing has been moved. For a security event, also notify the security officer within 24 hours.

5.After submitting

The supervisor will enter the event in the WHS Incident and Hazard Register and give you the register number. Keep a copy of the form. You will be contacted if an investigation is required.
